24-Hour Skilled Care
Each shift is staffed with licensed nurses and certified nursing assistants who receive continuous, on-going education and training. Our staff-to-resident ratio surpasses federal and state regulations. We believe in a resident-centered approach when delivering personal care at Huron County Medical Care Facility.
Activities
Our Activities Department provides daily group programming at all levels of functional ability. The activities are resident-oriented to meet their interests and preferences. In addition to daily programming, we offer monthly outings, many special events, and encourage and assist residents to pursue independent activities of choice. Large and small group activities are available. Residents are assisted to and from activities if by the activity staff when needed.

Pharmacy
Our consulting pharmacist conducts monthly reviews of the resident’s medical charts to ensure appropriate pharmaceutical levels. In addition to these regularly scheduled visits, quality assurance reports are prepared and discussed with the medical director, physicians, and nursing staff.
Social Services
The social workers at HCMCF understand that the transition from home to a long term care facility can be very difficult and challenging for our residents and their families. We are committed to easing that transition by providing emotional and social support during this adjustment period.

We also assist with discharge planning for those residents who are here for rehabilitation. We assist by providing you with the community resources you need to transition successfully back to the community. We work closely with local home-care companies, durable medical equipment companies, and Medicaid waiver programs.

Other duties include advocating for our residents to be sure your emotional and social needs are met and maintained during your stay. We work directly with Community Mental Health, Senior Wellness Group of Michigan, Probate Court, Hospice and many other agencies to meet your needs.

Transportation
The Huron County Medical Care Facility purchased a facility bus in 2008 to better serve residents. The purchase of the bus has greatly increased our flexibility to provide transportation to and from facility sponsored events, doctor appointments, etc. The facility vehicle is equipped with a lift for easy transportation of wheel chair bound residents.
